Work-Related Injuries

Employee Injury & Accident Information

If you experience a work-related injury, please inform your supervisor immediately. If the injury is serious or life-threatening 911 should be called. All injuries should be reported to a supervisor within 24 hours to ensure accurate documentation and workers’ compensation coverage.

NON-EMERGENCY

1. Contact your supervisor and report the incident
2. If medical treatment is required contact Kelly Dekoski at 591-8241 (if Kelly is not available contact Bonnie Crist 591-3212)
3. Complete an Employee Accident Investigation Report

If medical treatment is required, Kelly will complete the proper authorization forms and send you to:

Remember that, by law, treatment must begin with our worker compensation health care provider. Utilizing your own physician or healthcare provider for medical services may result in financial responsibility on your part.

How do you tell if it is an emergency? Listed below are a few indications of an emergency (this list is not all inclusive):

  • Deep lacerations
  • Breathing difficulty, with change in skin color
  • Chest pain not related to specific trauma
  • Caustic solution injury
  • Heat-related illness
  • Loss of consciousness
  • Broken bones which protrude through the skin
  • Amputation
  • Eye injuries with visual distortion or visible blood in eye
  • Burns over large areas of the body, or in which patient appears to be in SHOCK (cold skin, weak pulse, confusion, anxiety, tremor, dizziness)
